Implementing Cancer Survivorship Care Plans

To improve provider communication, follow-up care, and health outcomes for cancer survivors, many high-profile organizations (eg. the Commission on Cancer, the largest US cancer program accrediting organization) recommend or require cancer care providers to develop and deliver survivorship care plans (SCPs) - which ideally contain summary of treatment information, plans for follow-up, and recommended division of responsibilities among follow-up care providers - to survivors and their follow-up care providers. This webinar describes existing and planned research on implementing SCPs in practice.
